This form explains how funds may be used. 

 MMD Building Activities Documentation – use with Invoice #1
This form is completed by buildings and then submitted to the district contact or designee to compile results and complete the MMD District Invoice form. 

 MMD District Invoice #1
The original version of the invoice form has been updated to auto sum as the form is completed.  Only buildings that have completed all base activities can be recorded on this form.
